Get the embedded hook out the painlessly

1) place a strong piece of string or nylon behind the bend of the hook and double it.

2) hold the head of the fly firmly against the skin.

3) pull both ends of the string firmly in the direction the hook points went in.

The key is that you aren't pulling the hook out as much as you're pulling the flesh off the hook (while keeping the hook motionless).
